Community Services

Back to School Information & Resources

Norwalk parents: If your child is new to Norwalk Public Schools, entering Kindergarten, or otherwise due for their school physical next year, don't wait! Appointments may be harder to find later this summer, so check this task off your list now!

✅Make an appointment with your doctor. (Don't have a doctor? Check out our flyer for a list of wonderful community partners taking patients now!)

✅Keep your appointment, and ask for a school physical form (the "blue form") before you leave.

✅Submit the completed form to the Family Center this summer to make sure your child doesn't experience any delays in the Fall! (Please see the flyer for details.)

Not sure if your child needs a physical? Visit Norwalk Public Schools website for information: https://www.norwalkps.org/526507_3
